Project Worker – Creative Change Project, Brighton – THIS POST HAS NOW CLOSED

The Justlife Foundation, Brighton is looking to appoint a Project Worker to provide support to single homeless people living in Unsupported Temporary Accommodation, ensuring their housing vulnerability is as short, safe and healthy as possible.

The Creative Change project, part of Justlife, supports people vulnerably housed in Brighton & Hove. We have an exciting opportunity to appoint a new Project Worker who will provide support to people in emergency and unsupported temporary accommodation. Specifically, the work will involve taking on a caseload of homeless and vulnerably housed people and supporting them through a number of interventions including connecting clients with appropriate support services, accompanying them to appointments, assisting them in addressing wider health needs such as drug and alcohol use as well as needs around housing.

The role will also include promoting to your clients the Creative Change Project activities, such as drop in’s and workshops and other opportunities for people to engage in positive activities, build a sense of community and develop self confidence to improve chances of health, housing and employment.

We are looking for an Project  Worker who is highly motivated and has some experience of working in a related field in a voluntary or paid capacity.

Engagement Worker – Pathway Plus, Brighton – THIS POST HAS NOW CLOSED

Pathway Plus is a pioneering project, funded by Brighton & Hove CCG as part of the integrated care programme. The project aims to improve the access and engagement with healthcare of those who are homeless or vulnerably housed and will work closely with specialist homeless healthcare provider Arch Health CIC.

We have an exciting opportunity to appoint 2 new Engagement Worker’s who will provide support to people experiencing housing vulnerability to access and engage with health care providers. The project works with people upon discharge from hospital, or as referred by GP surgeries or the voluntary sector, to ensure that their health recovery is sustained and that they receive relevant assistance with their broader needs, linking in with the wide range of services which are available in the city. Specifically, the work will involve liaising with stakeholders and taking on a caseload of homeless patients at the point of discharge from hospital or referral. Once referrals are received, appropriate care plans will be established and this team will provide non-clinical floating support to individuals.

Regular activities will be connecting clients with appropriate support services, accompanying them to appointments, assisting them in addressing wider health needs such as drug and alcohol use. Staff will also be involved in the wider work of Justlife in Brighton and Hove providing support to people who live in emergency accommodation and vulnerably housed situations.  We are looking for an Engagement Worker who is highly motivated and has some experience of working in a related field in a voluntary or paid capacity.
