PathwayPlus set to continue in Brighton and Hove

pathway-logoWe are pleased to announce that the Brighton and Hove Clinical Commissioning Group (CCG) have recommissioned the PathwayPlus Hospital Discharge Project which Justlife have been piloting with partners, St. John Ambulance, Brighton Homeless Healthcare and Pathway, since October 2013.The initial pilot was funded through Department of Health until March 2014 but we are delighted now to have secured funding for all four partners until March 2015 which will give us time to establish some serious and credible evidence regarding the economic and social impact of the project on the homeless community.The Justlife team provides support to patients who live in temporary accommodation when they are discharged from hospital to ensure that their health recovery is sustained and that they receive relevant assistance with their broader needs, linking in with the wide range of services which are available in the city. Once referrals from Pathway are received, appropriate care plans will be established and this team will provide non-clinical floating support to individuals who live in temporary accommodation.The Justlife team work hard to deliver the PathwayPlus project and we are pleased that they will all be remaining in post. The PathwayPlus team is led by Simon Tanner, Patient Co-ordinator. The project workers are Fran Byron and Tom Jayston.Gary Bishop, CEO of Justlife said: ‘I’m absolutely delighted that for at least the next 12 months people who are leaving hospital and being housed in temporary accommodation will have the kind of support that they need to continue their health recovery and improve their future prospects. This is an important step for us and we are really looking forward to working, once again with the excellent PathwayPlus partners to serve the city in this way.’
